mostly using Adobe PhotoShop and ImageReady. i've read that you can do it with The GIMP, which is free, but I haven't really tried to figure it out since I have a fully legal licensed copy of the Adobe Design Suite.

if you're using photoshop and imageready, get all the frames set as layers or combinations of layers in your photoshop document, then move it to imageready (there's a button at the bottom of the toolbar that does this), and my recollection is that it's fairly straight-forward to make an animated gif from the layers once you're in imageready.
